Note:
1. Copy to your
computer.
2. Make changes to
agree with your office and/or instructor.
3. Move the "X" up
or down two lines to agree with either Insurance or Points.
4. When using for
the first time print out on a blank sheet of paper. Place it on top
of a certificate, hold it up to the light and make sure it will print on
each line correctly.
